  1. Do you have a homelessness policy?
  2. Has a homeless health needs audit been completed?
  3. Has a local homelessness review been completed to consider health and wellbeing and the role of health services?
  4. Do you provide training for those working with people experiencing homelessness in health care services, for example, standards for GP receptionists in primary care and practice managers?
  5. Do you have Pathways / protocols / services in place to prevent homelessness at discharge from hospital (including mental health and on release from prison)?
  6. Do you have a multidisciplinary team that works with people who are homeless? 

Response

  1. Do you have a homelessness policy?
    • We have a joint south Nottinghamshire Homelessness and Rough Sleeping Strategy - this is statutory requirement (can be found on the website)
  2. Has a homeless health needs audit been completed?
    • Nottinghamshire County Council has undertaken a homelessness Joint Strategic Needs Assessment (JSNA) for homelessness.
  3. Has a local homelessness review been completed to consider health and wellbeing and the role of health services?
    • County Council JSNA as above.
  4. Do you provide training for those working with people experiencing homelessness in health care services, for example, standards for GP receptionists in primary care and practice managers?
    • This will be part of the JSNA action plan
  5. Do you have Pathways/protocols/ services in place to prevent homelessness at discharge from hospital (including mental health and on release from prison)?
    • We have been discussing these at county meetings but as yet we don't actually have a developed protocol for either, although Mansfield District Council has a hospital discharge scheme. We get notified through the Duty to Refer requirement.
  6. Do you have a multidisciplinary team that works with people who are homeless? 
    • No
