Rushcliffe working together to become a Dementia Friendly Community

The Rushcliffe Dementia Communication and Support Project is a two year initiative which aims to improve the support available to people living with dementia and their carers.  

The aim of the project, delivered by Rushcliffe Primary Care Network in partnership with Rushcliffe Borough Council, Nottinghamshire County Council plus many community and voluntary organisations, is to enable Rushcliffe residents living with dementia and their carers, to live better, more independent lives for longer, through easier access to timely, high quality, individualised support and signposting.

In June 2023, Rushcliffe Primary Care Network’s Rushcliffe Dementia Communication and Support Project was named as the Regional Champion in the ‘The Excellence in Healthcare Award’.

Living with Dementia - The Dementia Wellbeing Service

The Alzheimer's Society Dementia Wellbeing Service offers a variety of resources including telephone advice and support or a request for a home visit as part of the Dementia Support Service. Call 0333 150456 or visit the Dementia Wellbeing Service website.

Nottinghamshire Carers Hub - Support for Carers

Nottinghamshire Carers Hub provide free, confidential and personalised information and support for unpaid carers.

Hidden Disability Lanyards

The Hidden Disabilities Sunflower is a simple tool for you to voluntarily share that you have a disability or condition that may not be immediately apparent. Sunflower lanyards have become a popular way for organisations to spot if someone has a non-visible disability, such as dementia. They’re available at many major retail outlets or to order lanyards online.

Rushcliffe Health and Wellbeing Social Prescribing Dementia Directory

Social prescribers are part of your GP Practice Team and are there to offer practical support and signposting from a non-medical perspective. Their Dementia Directory is a one-stop-shop for support and advice. There’s information on local groups such as memory cafes to community transport as well as respite support for carers. Green Memory Café

Working in partnership with Rushcliffe Dementia Project and Rushcliffe Primary Care Network, the Green Memory Café takes place at Rushcliffe Country Park bi-monthly for people living with memory loss and their carers.

Rushcliffe Big Green Book

The Rushcliffe Big Green Book is a part of the Nottingham Community Voluntary Service  GreenSpace Green Social Prescribing Project and is directory of nature-based activities and opportunities around Rushcliffe.

Green Social Prescribing is a way of connecting people to nature-based activities and green groups, projects and schemes in their local community to support with health and wellbeing. Physical copies of the book are accessible in local GP surgeries and libraries.

The book is now delivered in partnership with Rushcliffe Borough Council and Rushcliffe Primary Care Network  and sets out local activities and groups across the Borough to help people choose an activity that is local and easily accessible to them.

The Rushcliffe Big Green Book is working in collaboration with the Rushcliffe Dementia Friendly project to enable our communities to be more inclusive. For more information, please refer to Page 4 in the Rushcliffe Big Green Book.

Rushcliffe Dementia Action Network (RDAN)

Rushcliffe Dementia Action Network is a partnership which brings together local groups and organisations to improve the lives of people living with Dementia and their carers.

Dementia Friends Information Session

Dementia Friends is a light touch information awareness session with a mission to educate, reduce stigma and spread knowledge about dementia. Become a Dementia Friend by accessing a free Information Session online or in person through The Alzheimer’s Society Dementia Friends or call 0333 150 3456.

Carer Awareness Training

Gain the Carer Friendly Quality Mark for you and/or your organisation. This can be offered virtually or in person, organised through Nottinghamshire Carers Association or email: info@nottinghamshirecarers.co.uk

Signposting

Across Rushcliffe we have created a Population Health Management Calendar with each month focusing on a health category to promotes services and self-help resources available to anyone. Find resources on what you can do to improve your health.

The Social Prescribing Team have put together a full, easy to understand, list of directories for health and social care conditions. Each one is filled with tips, links to websites offering help, and information about services offering support on the issue.
