• Licence number
• Operator name
• Type of Licence*
• Star rating
• Licence expiry date
• Number and type of animals
   
• Address Line 1
• Address Line 2
• Address Line 3
• Postcode, Part of Postcode, Ward or town name 

*Type of Licence
 
This should be one of:

• Commercial boarding cats
• Commercial boarding dogs
• Home boarding dogs
• Dog day care/Arranger
• Dog breeding
• Hiring of Horses
• Exhibition animals
• Selling as pets
